AAC- 12 The care shall be provided by expanding access to health practices through domiciliary visits, where ever applicable 

Scope: Providing care/ follow up to the patients discharged  from DMC&H, HDHI or DMCH Cancer centre with trained staff.

Responsibility: Administration/ Nursing Head/ Home care

Services Provided:

 Nursing support: It provides various following procedures and care related assistance from qualified nurses.

      • IV cannulation

      • I/V Drug administration

      • Urinary Catheter care

      • Urinary Catheter change

      • Ryle’s Tube feeding

      • Ryle’s Tube insertion

      • Stoma Care

      • Stoma Bag change

      • Tracheostomy Care

      • Bedridden Patient Care

      • Enema

      • Wound Dressing

      • Pressure ulcer care & dressing

      • Care of patient on Ventilatory management (BiPAP/ CPAP/ Tracheostomy)

      • Nebulization

      • Emotional as well as Psychological support

Physiotherapy Support: Qualified physiotherapist provide  therapeutic physical treatments and exercises as per the need & recommendations at the time of discharge. 

Dietician Support: Qualified dieticians provides dietary  advices for various category of patients as per their dietary need such as gastrointestinal issues, malnutrition, weight management and home enteral feeding for RT feed on consultation.

Attendant Care: Supportive care provided by trained attendant as per the given details:

      • Bathing assistance

      • Feeding (oral feed) 

      • Walking assistance

      • Medicine reminders

      • Mother and Baby Care

      • Care for patient living alone

      • Care for patient with Dementia or Alzheimer Disease

      • Monitoring vital signs

      • Nebulization

      • Care for bed ridden patients

Specialized unit care: Nurses working with specializedunits such as cardiology, cardiac surgery or cardiac critical care guide and coordinate the care of patients discharged from these units on telephone. 

Infection control care: Infection control nurse monitors all the patients discharged with urinary catheter or other catheters for monitoring telephonically. Also ICN monitoring all patients discharged with devices, implants or CABG for follow up related with infection at surgical site.

Communication & guidance in unusual situation

During the war or pandemic situations telephonic communication with all cardiac patients including OPD patients is done for continuity of their care and treatment.
